Can somebody confirm that the form to use in applying for British Naturalisation for a family member of EEA national who is holding a permanent residence card is FORM AN? I just passed the Life in the UK test exam. Thanks.

Yes, form AN is the correct form this category. There is a column in the form which asks for date of grant of ILR. In this column, mention the date of issue of permanent residence card. Also note that since you already have PR (which is equivalent for ILR) you do not need to submit proof of exercising treaty rights again. Form AN is a bit confusing abt this.
