I want to get my Spouse extened can you tell me the came of the form is it called FLR (M) ???????????????????

How long can i extend it for ????????????????

Yes, FLR(M). All the information is there if you read the form and relevant information. Current financial requirement of minimum £18600 income will not apply to you.

Any specific reason why you want to extend you visa rather than applying for ILR?
