You only do test once. Once you have done the test for Indefinite leave to remain, you do not need to do any other test again when you apply for nationality. On the nationality application form AN, you will just tick on the box saying that you have already done the test at the time of ILR.

You should first clarify under which immigration category are in. What do you mean by leave to remain visa? Have you applied for Indefinite leave to remain?
