I had a read of section 47, and it appears what you have been served is regarded as section 10 immigration act 1971, which in turn means IS151A.

Just tick ISI51A and then explain the scenario in additional information when you submit your application.

Also note, do not apply online from UK otherwise your application will be invalid. You can start the application online from UK, but the final send button must only be pressed when you are physically in the country you are applying from.

Just to update this post...

I had to submit my application before hearing from you and, although the refusal letter had mentioned that a decision had been made to remove me from the UK, I ticked that I was not "required" to leave the UK; because I consulted dictionary on the verb "require" and it was mentioned that it has to be through a a formal request while I was not formally asked to leave the UK. My previous visa was expired before the receipt of the refusal letter and by law, I had a limited, definite time to leave the UK - which I did.

Therefore, I ticked I was not removed, deported or required to leave the UK and I have been granted visa.

I ought to thank "mr lookforward" for his advices in filling in my application. Thank you.

Hello,
I have been refused extension on my leave to remain and the letter says that a decision has been made to remove me from the UK. I haven't received any other letter or notice. I didn't appeal and am going out of the country to re-apply - what I should have done from the beginning. My disqualification was because I had applied in the country while I shouldn't.
Now that I am filling the on-line form of application, it asks which one of these have been served to me: deportation order, APP104 or IS151A while my refusal letter doesn't contain any of these. So, what I should do? If I say no to the question answering if I have been required to leave the UK, I have given wrong info and if I say yes, I must choose one of these. Please do advise.

Have I ever been "required to leave" considering the following statements??

In a part of my letter it says:
A decision has also been made to remove you from the UK by way of direction under section 47 of the Immigration Asylum and nationality Act 2006.

At the end of the refusal letter:
If you choose not to appeal this decision, or you appeal and the appeal is unsuccessful, you must leave the UK immediately. If you do not leave the UK voluntarily, you will be removed.
