you need to fill new from, and make sure you send 3 months bank statements and 3 payslips along...

Not sure if you are on a work permit, but if you do, you need aletter from the employer stating that you will be employed with them in the future.

                         I am going next month to apply ILR so can you tell can I send all last three month identification documents like bank statement etc.. in the home office or just only one month.SECOND I can fill new form for ILR or old form.
