You can get the form:
1. Calling/Writing the Home Office and they will send you with Instructions. I sent the request 2 months ago so they might be coming very soon. I can also send you those if they arrive.
2. Downloading from the Internet.
3. Go to the Law Centre that helps with Immigration Issues.
4. I am sure not 100% but Nationality Checking Service can provide these forms also. Call them in your area
5. Go to the Library and print the form if you don't have Internet or the printer. Do not print the instructions it's too large and if Library charges per page it will be expensive.
They have a Legal Help Centre in Bradford, they provided me Form and Instructions free. I returned the instructions so someone else can use it and donated my Life in The UK book to their centre so others can borrow it to take the test.
