The requirements for naturalisation are fairly straightforward, I think, and one of the requirements is that the applicant must have been in the UK "legally" (ie with permission) throughout the five year residence period. Any gaps between visas can be taken into account, and if there is a gap, this can result in the clock starting again. I don't know
nexhi salhu's circumstances, but it sounds to me as though the Home Office have identified such a gap, and that they are saying that his/her clock restarted as a result of it, some time in 2004.
It is impossible to tell whether their reconsideration of the application will result in its succeeding this time or not. But it seems to me that it is definitely worth taking the test, in order to give it a chance. Poor health can certainly be taken into account in excusing someone from taking the test, and it sounds as though they accepted that back in April but if it wasn't a permanent problem, I think they are justified in asking you to take the test now.
Bear in mind that a successful test result does not expire, so even if you take it and then they refuse your application anyway, you will still have the "pass" certificate to submit when you apply again in 2009. So I'd say, go for it.... Looking through the forums here, you can see plenty of posts from people who have passed it, so I don't think it is anything to be too nervous about.
